    Late reoperation in vascular surgery

    Objectives:Assessment of late reoperation (after 30 days) following vascular surgery.Design:Analysis of a prospectively collected database of consecutive patients undergoing vascular surgery.Setting:A single teaching unit's experience between 1986–1993.Materials:Patients undergoing 2501 primary arterial reconstructions.Chief outcome measures:Reoperation after 30 days.Main results:One hundred and fifty eight patients (6%) underwent further operations, at more than 1 month after the primary procedure. Primary procedures at highest risk for reoperations were axillobifemoral bypasses and femorodistal bypasses with respective late reoperation rates of 20% and 16%. The majority of patients required late reoperation because of graft occlusion or stenosis. Overall, of the 158 late reoperations performed, 114 were related to the same arterial segment with the same presenting symptoms as the primary operation, and 44 for a different indication. A second or subsequent reoperation was required in 54 patients and the overall operative mortality was 11%.Conclusion:Patients undergoing certain vascular procedures, should be informed of the high risk of a subsequent procedure when consent is obtained

    A pragmatic randomized trial comparing tablet computer informed consent to traditional paper-based methods for an osteoporosis study

    AbstractObjectiveMethods to improve informed consent efficiency and effectiveness are needed for pragmatic clinical trials. We compared informed consent using a tablet computer to a paper approach to assess comprehension and satisfaction of patients and clinic staff for a future osteoporosis clinical trial.MethodsNine community-based practices identified and recruited patients to compare the informed consent processes (tablet vs. paper) in a mock osteoporosis clinical trial. The tablet informed consent included an animation summarizing the trial, complete informed consent document, and questions to assess and reinforce comprehension of the study. Participants were women age ≄55 years with ≄1 year of alendronate use. We surveyed participants to assess comprehension and satisfaction and office staff for satisfaction and perceived time demands.ResultsThe nine practices enrolled 33 participants. There was not a significant difference in comprehension between the tablet vs. paper informed consent [mean (SD) tablet: 12.2 (1.0) vs. paper: 11.4 (1.7)]. Office staff preferred the tablet to the paper informed consent for identifying potential study participants (two-sided t-test p = 0.02) despite an increased perceived time spent to complete the tablet process [tablet: 28.3 min (SD 16.3) vs. paper: 19.0 min (SD 6.9); p = 0.08].ConclusionsAlthough, there were no significant differences in participant satisfaction and comprehension with the tablet informed consent compared to a paper informed consent, patients and office staff trended towards greater satisfaction with the tablet informed consent. Larger studies are needed to further evaluate the utility of electronic informed consent in pragmatic clinical trials

    Synthesis, biological profiling and mechanistic studies of 4-aminoquinoline-based heterodimeric compounds with dual trypanocidal–antiplasmodial activity.

    YesDual submicromolar trypanocidal–antiplasmodial compounds have been identified by screening and chemical synthesis of 4-aminoquinoline-based heterodimeric compounds of three different structural classes. In Trypanosoma brucei, inhibition of the enzyme trypanothione reductase seems to be involved in the potent trypanocidal activity of these heterodimers, although it is probably not the main biological target. Regarding antiplasmodial activity, the heterodimers seem to share the mode of action of the antimalarial drug chloroquine, which involves inhibition of the haem detoxification process. Interestingly, all of these heterodimers display good brain permeabilities, thereby being potentially useful for late stage human African trypanosomiasis. Future optimization of these compounds should focus mainly on decreasing cytotoxicity and acetylcholinesterase inhibitory activity

    Benchmark Parameters for CMB Polarization Experiments

    The recently detected polarization of the cosmic microwave background (CMB) holds the potential for revealing the physics of inflation and gravitationally mapping the large-scale structure of the universe, if so called B-mode signals below 10^{-7}, or tenths of a uK, can be reliably detected. We provide a language for describing systematic effects which distort the observed CMB temperature and polarization fields and so contaminate the B-modes. We identify 7 types of effects, described by 11 distortion fields, and show their association with known instrumental systematics such as common mode and differential gain fluctuations, line cross-coupling, pointing errors, and differential polarized beam effects. Because of aliasing from the small-scale structure in the CMB, even uncorrelated fluctuations in these effects can affect the large-scale B modes relevant to gravitational waves. Many of these problems are greatly reduced by having an instrumental beam that resolves the primary anisotropies (FWHM << 10'). To reach the ultimate goal of an inflationary energy scale of 3 \times 10^{15} GeV, polarization distortion fluctuations must be controlled at the 10^{-2}-10^{-3} level and temperature leakage to the 10^{-4}-10^{-3} level depending on effect. For example pointing errors must be controlled to 1.5'' rms for arcminute scale beams or a percent of the Gaussian beam width for larger beams; low spatial frequency differential gain fluctuations or line cross-coupling must be eliminated at the level of 10^{-4} rms.Comment: 11 pages, 5 figures, submitted to PR

    We study the classical approximate string matching problem, that is, given strings PP and QQ and an error threshold kk, find all ending positions of substrings of QQ whose edit distance to PP is at most kk. Let PP and QQ have lengths mm and nn, respectively. On a standard unit-cost word RAM with word size w≄log⁥nw \geq \log n we present an algorithm using time O(nk⋅min⁥(log⁥2mlog⁥n,log⁥2mlog⁥ww)+n) O(nk \cdot \min(\frac{\log^2 m}{\log n},\frac{\log^2 m\log w}{w}) + n) When PP is short, namely, m=2o(log⁥n)m = 2^{o(\sqrt{\log n})} or m=2o(w/log⁥w)m = 2^{o(\sqrt{w/\log w})} this improves the previously best known time bounds for the problem.     Galaxy And Mass Assembly (GAMA): detection of low-surface-brightness galaxies from SDSS data

    We report on a search for new low-surface-brightness galaxies (LSBGs) using Sloan Digital Sky Survey (SDSS) data within the Galaxy And Mass Assembly (GAMA) equatorial fields. The search method consisted of masking objects detected with SDSS PHOTO, combining gri images weighted to maximize the expected signal-to-noise ratio, and smoothing the images. The processed images were then run through a detection algorithm that finds all pixels above a set threshold and groups them based on their proximity to one another. The list of detections was cleaned of contaminants such as diffraction spikes and the faint wings of masked objects. From these, selecting potentially the brightest in terms of total flux, a list of 343 LSBGs was produced having been confirmed using VISTA Kilo-degree Infrared Galaxy Survey (VIKING) imaging. The photometry of this sample was refined using the deeper VIKING Z band as the aperture-defining band. Measuring their g − i and J − K colours shows that most are consistent with being at redshifts less than 0.2. The photometry is carried out using an AUTO aperture for each detection giving surface brightnesses of ÎŒr ≳ 25 mag arcsec−2 and magnitudes of r > 19.8 mag. None of these galaxies are bright enough to be within the GAMA main survey limit but could be part of future deeper surveys to measure the low-mass end of the galaxy stellar mass function

    Galaxy And Mass Assembly (GAMA): the signatures of galaxy interactions as viewed from small scale galaxy clustering

    Statistical studies of galaxy–galaxy interactions often utilize net change in physical properties of progenitors as a function of the separation between their nuclei to trace both the strength and the observable time-scale of their interaction. In this study, we use two-point auto-, cross-, and mark-correlation functions to investigate the extent to which small-scale clustering properties of star-forming galaxies can be used to gain physical insight into galaxy–galaxy interactions between galaxies of similar optical brightness and stellar mass. The H α star formers, drawn from the highly spatially complete Galaxy And Mass Assembly (GAMA) survey, show an increase in clustering at small separations. Moreover, the clustering strength shows a strong dependence on optical brightness and stellar mass, where (1) the clustering amplitude of optically brighter galaxies at a given separation is larger than that of optically fainter systems, (2) the small-scale-clustering properties (e.g. the strength, the scale at which the signal relative to the fiducial power law plateaus) of star-forming galaxies appear to differ as a function of increasing optical brightness of galaxies. According to cross- and mark-correlation analyses, the former result is largely driven by the increased dust content in optically bright star-forming galaxies. The latter could be interpreted as evidence of a correlation between interaction-scale and optical brightness of galaxies, where physical evidence of interactions between optically bright star formers, likely hosted within relatively massive haloes, persists over larger separations than those between optically faint star formers

    Scalar field exact solutions for non-flat FLRW cosmology: A technique from non-linear Schr\"odinger-type formulation

    We report a method of solving for canonical scalar field exact solution in a non-flat FLRW universe with barotropic fluid using non-linear Schr\"{o}dinger (NLS)-type formulation in comparison to the method in the standard Friedmann framework. We consider phantom and non-phantom scalar field cases with exponential and power-law accelerating expansion. Analysis on effective equation of state to both cases of expansion is also performed.     A series of polyurethane elastomers based on an aliphatic diisocyanate and a polyether macroglycol was polymerized with various crosslink densities and OH/NCO ratios.
